Overview

Thank you for your interest in joining the Ascend Network! To join this free network, please fill out the following survey and follow the instructions provided at the end. All individual responses will be kept confidential by Ascend staff. We appreciate your time and commitment. Thank you!

The purpose of this survey is to better understand your areas of focus, particularly in relation to the 2Gen approach, in order to best identify areas of support and connection across our Network in service of accelerating 2Gen systems change. We recognize that organizations are at different stages of 2Gen implementation, and your work is complex. Please answer the questions honestly based on your current practices and goals—there are no “right” or “wrong” answers. Your responses do not affect your eligibility to join the Ascend Network.

This survey should take 15 minutes to complete and consists of three sections:
  • Organizational Overview - To describe your work’s mission, structure, and target population.
  • 2Gen Journey - To detail your current 2Gen progress, goals, and barriers.
  • Network Profile - To input contact information for future engagement and resource sharing.
