This survey should take 15 minutes to complete and consists of four sections:
  1. 2Gen Primer - To ground provide a brief overview of 2Gen approaches.
  2. Organizational overview - To describe your work’s mission, structure, and target population.
  3. 2Gen Progress - To detail your current 2Gen progress, goals, and barriers.
  4. Profile - To input contact information for future engagement and resource sharing.
The Ascend Network is a community of practitioners, researchers, institutions, and systems and policy leaders keenly focused on documenting their 2Gen successes and challenges and sharing their lessons and outcomes.

The purpose of this survey is to better understand your organization’s 2Gen work and focus areas in order to identify areas of support for your organization; connect you to others in the Network; and accelerate your policy and systems change efforts. We recognize that organizations are at different stages of 2Gen implementation, and your work is complex. Please answer the questions honestly based on your current practices and goals—there are no “right” or “wrong” answers. Your responses do not affect your eligibility to join the Ascend Network.
